Please read this form carefully; it is intended to assist you in selecting a cat that best suits you, your family and your lifestyle. Applicants MUST be 21 years of age to apply. All questions MUST be answered completely. Incomplete applications may cause delay and/or non-approval of your application. Please be advised that applications are processed in the order in which they are received. All information, including vet and personal references will be contacted and verified. Due to the high volume of applications we receive each week, only those applicants who have been approved to adopt will be contacted. Please note: for liability reasons we will not adopt to families with young children under the age of 7. 

We highly recommend you contact your vets office (past/present) to inform them you are applying to adopt a new pet from Friends of the Plymouth Pound; and that a rep from FPP will be contacting them to verify medical information about your pets. Many vets will not disclose this information without your consent.

A $150 adoption fee is collected at the time of adoption. This fee is deposited into our animal care fund to help defray the high cost of veterinary care provided to the animals in our rescue/adoption program. Please understand that cats require vet care throughout their entire lifetime such as annual exams, vaccines, flea/tick preventative etc. so please consider this financial expense when thinking about adoption. As part of the Friends of the Plymouth Pound Adoption Contract you sign, you agree and are required to contact FPP immediately if you are no longer able or willing to care for the cat you adopted (for any reason during it's lifetime), and the cat is to be returned to FPP.  By submitting this application you agree and understand to the terms as stated above.
